Case Manager

I am recruiting for a new Case Manager to join a Case Management specialist who provide Case Management services for adults and children across the North West and North Wales.
I am specifically looking to add to both the children's team and the adult's team, who provide support to adults and children who have either ABI and complex needs from catastrophic injuries or birthing injuries.
The role offers hybrid working, working from home being an option, with 1 day a week at least in the office. In addition, there would be an expectation with training and shadowing to travel to the office, which is Wirral based.
Duties:

• Work within a team of experienced social care professionals
• An experienced Case Manager will eventually have a caseload of 5, or more, dependant on the needs of each package
• Managing clients case management needs across North Wales and North West.
• Coordination of health, care, rehabilitation, educational, and employment services for your clients.
• Meet potential clients
• Ensure all practices fall in line with CQC standards
• Ensure all notes are detailed and record necessary details
• Create and manage cases for new clients
• Time management, ensuring all cases get attention which is required
• Liaise and build rapport with families of clients
• Arrange, and update clients on meetings, appointments
• Ensure appointments and meetings are accessible and run smoothly
• Meet and communicate with clients and families
• Report writing
• Work within CQC standards

Types of services to be arranged include: arranging and organising care, adaptations to house and work places, organising and managing travel to work and holidays, organising ad-hoc support, organising rehabilitation etc.
This is an extremely rewarding role, no two days are the same, offering you the autonomy to plan your day, ensuring all tasks are completed.
Pay and Hours:

• £35,000 - £47,000 - Dependant on experience, the top end would be for experienced Case Managers
• Full-time, permanent contract
• Monday - Friday, 9-5, with an hour lunch break - flexibility needed.

Requirements

• 2 years post qualification experience as a Social Worker
• Driving Licence
• Ideally, experience as a Case manager, but not essential
